We will be taking on New Patients who have moved into the area from 1st May 2021 – 31st May 2021. We are unable to register patients who are currently registered with a GP practice in Musselburgh
Important: Unfortunately any forms that are submitted before 1st May or After 31st May are unable to be processed.

Please download and complete the Inveresk Medical Practice new patient form (Please note there is a separate form for children):

On receipt of the email we will email back to confirm receipt and if your registration has been accepted or not, e.g. practice is not taking on patients that month, or you live outwith the practice area etc.
If you are unable to download the forms please contact our reception team who will assist you further by posting a registration pack to you
Acceptance on to the list is as at the discretion of the Doctor. We will ask you for proof of identity. Once you have been accepted as a patient, your medical records will be transferred to the Practice. Your medical records may take several weeks to arrive at the Practice. In the rare event of a patient being violent or abusive, the patient will be removed from the practice list. The patient will be informed of the reason for removal in writing and the Health Board will be informed.
Please supply two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse care. Overseas visitors may be asked for additional information to prove entitlement. One form of identity must ideally be photographic such as a Passport, Drivers Licence, Official ID card from Public Services body, Student matriculation card current year)
Registration Policy
In accordance with Health Board name recommendations, all new patients should present two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.
One form of identity must ideally be photographic such as:
- Passport
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
Other documents for proof of residency that are acceptable are:
- Recent utility bill (within last 3 months)
- Council Tax document
- Television Licence
- Payslips (last two months)
- Rent book/agreement (Public Body or Private Landlord)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- Divorce Annulment Papers.
N.B. The above list is not exhaustive and staff are entitled to ask for further proof if required. Please note you will not be registered with the practice if you cannot provide the documents required to prove NHS eligibility.
